Important! It is the customer's responsibility to ensure that a copyright release is obtained prior to submission for orders requesting to add a photo that was taken by a professional photography studio. Most photography studios will supply this form upon request without hesitation.

Once you obtain this form and it is completely filled out and signed by the photographer, send it to us ASAP by fax, Email, or mail so as to not delay the processing of your order.

If the photo was not professionally taken, no form or any kind is needed.

If e-mailing your photo, please be sure that photo is scanned at a resolution of 300 dpi and saved as a JPG or JPEG file type. Please do not edit or crop photos. Please include your order number with your attached photo. You will receive your confirmation order number after you have placed your order. Should you decide to mail us your photo, send it Sweetly Wrapped Occasions, PO Box 271480 Las Vegas, NV 89127. We strongly recommend that you use a photo mailer. Mailed photos will be returned to the sender with the order or separately in mail.
